What’s Behind Fatigue in EDS/HSD

Fatigue in EDS (Ehlers-Danlos syndrome)/HSD (hypermobility spectrum disorder) isn't just feeling tired. It's exhaustion that wraps around your body like dense fog. It can be there when you wake up, and even when you're doing nothing. Here’s what can be behind it:

Muscle Overwork

Your muscles may be doing extra work just to hold you together.

Pain

Chronic pain is draining to the body. Even when you're not focusing on it, your body may be spending energy on it.

Cognitive Load

Constantly managing your body—feeling like you have to watch every step and every movement—takes a mental toll.

Autonomic Dysfunction (like POTS)

When blood doesn’t circulate properly, crushing fatigue after prolonged standing or activity can result.

Poor Sleep Quality

Even if you’re clocking hours, things like pain, sleep apnea, mast cell activation syndrome (MCAS), or dysautonomia can impact restorative sleep.

Frustration & Isolation

When people don’t see your fatigue, they may assume you’re lazy, unmotivated, or complaining. That emotional weight adds up too.

This isn’t your average tired. Living with EDS/HSD, not to mention some of the other conditions it can present with, means your body’s working overtime just to stay together. It’s the backdrop of every moment, whether you’re moving or still.
